Terms and conditions for Unlimited Monthly Membership SS + SB

We are glad you chose us for your fitness needs and hope you enjoy all the programs that we offer. Our Stroller Strides + Stroller Barre classes are a great way to get in shape, meet other moms and their children and to gain support as a parent. Below are our policies, terms and conditions as well as how to contact us with any questions or concerns you may have regarding your membership.

STROLLER STRIDES + STROLLER BARRE MONTHLY MEMBERSHIP AGREEMENT:

It is recommended that you wait 6 weeks after the birth of your baby to start Stroller Strides. ALWAYS get your physician’s approval before beginning any exercise regimen. By accepting these terms, you are agreeing that you have been cleared by your physician for physical activity. %%INITIAL%%

This contract made between the client and FIT4MOM Temecula Valley is a month to month agreement and except as provided in the initial 10-day cancellation policy below, may be terminated by either party with the proper advanced notice. This membership allows you to attend unlimited Stroller Strides, Stroller Barre, and Strides 360 classes. %%INITIAL%%

AUTHORIZATION FOR PAYMENT
I authorize FIT4MOM Temecula Valley to initiate a charge to the credit card number entered into Pike13 for my dues on the 1st day of every month. This authorization is to remain effective until I have cancelled my membership through the FIT4MOM franchise owner (see cancellation policy).

I have the right to stop payment on an Electronic Funds Transfer debit by notifying my bank. This, however, does not void my contract with FIT4MOM Temecula Valley to fulfill my payment commitment and I am obligated to pay. Dues will be drawn monthly from the account on file on the first of the month. The processing date for debit cards may vary due to banking procedures. If charges are returned they will be subject to a return fee of $25. Members are responsible for updating their card information online. Members may not attend class who have unpaid balances. If a membership remains unpaid it will be cancelled. If in the future you wish to return, all dues unpaid from previous membership will be due in addition to the new membership fees. %%INITIAL%%

COMMITMENT/CANCELLATION POLICY
This plan requires a 3-month commitment. If you wish to cancel before this, all dues owed for the 3 months will be charged to the card on file. %%INITIAL%%


To cancel this membership, the member must contact the FIT4MOM franchise owner at least 30 days prior to their next payment due date to request a cancellation and then fill out the cancellation form (link will be provided by Melissa Merrill). IF you do not notify before 30 days, a $45 cancellation fee will be charged. No refunds will be given for dues already paid. This membership is not transferable to another person or anywhere outside the FIT4MOM Temecula Valley franchise. Once a membership is cancelled, if the member wants to re-enroll they will be charged at the current plan price and a new enrollment fee may be charged. %%INITIAL%%


HOLD POLICY
We have a three month hold policy. All hold requests MUST be submitted 7 days BEFORE the next billing cycle. There is a $20 fee associated with the hold placed. If you choose to place your account on hold and it is less than 7 days before the billing cycle, 50% of the membership dues will be owed. Once submitted, a confirmation will be sent to you via email. A hold does not subject you to deferral of commitment upon returning. If you wish to cancel before the hold ends, there is a $45 cancellation fee. %%INITIAL%%

INITIAL 10-DAY CANCELLATION POLICY
You, the buyer, may cancel this agreement at any time prior to midnight of the tenth (10th) calendar day after you have signed this agreement. To cancel this agreement, written notice must be received by FIT4MOM Temecula Valley by email within the initial ten days. You are responsible for paying for the classes already attended. %%INITIAL%%

SAFETY/STROLLER POLICY
-Only children age 5 (kindergarten) and older may be out of the strollers during class. %%INITIAL%%
-Children not in the stroller (5+) may travel by scooter with a helmet, or run alongside mom. When we are parked, so must their scooters be parked

-All older children MUST be seated during workout stations. %%INITIAL%%
-All stroller-aged children MUST be buckled in their strollers during the hour class.%%INITIAL%%
-The resistance/mini bands are for the use of the adults only during class. %%INITIAL%%

We understand that there may be times that your child(ren) may want to get out during class, but this is for their safety, the safety of the moms as well as the instructors that they remain in their strollers. By agreeing to this, you take FULL responsibility for any injuries that may occur if your child is out of their stroller during class hours or any injuries that may occur if your child is over 5 and walking/scootering etc... during class. %%INITIAL%%

VISITORS
Have family or friends in town? Is your significant other off of work for the day? Bring them to class FREE of charge! If you have an extended stay guest who will participate longer than one week, they will be subject to a $5 per class charge. Please talk to the instructor(s) to let them know you will have an out of town visitor joining you. %%INITIAL%%

BANDS
This membership INCLUDES a red SKLZ resistance band once the $45 enrollment fee is paid. Please know, YOU are responsible for caring and maintaining your resistance bands. Check regularly for any damage to the bands. IF you are in need of new resistance band(s), please contact Melissa Merrill for a new band ($20). %%INITIAL%%

WEATHER POLICY
We will send an email to all enrolled participants on Pike13 and post on our social media accounts (including the private Facebook page) of any class changes. Class will not be cancelled due to weather. We will move class onto our FIT4MOM Virtual Studio via Zoom. Please be sure to create a zoom account for days that may be too hot/cold/rainy for outdoor classes. %%INITIAL%%

PRICING/POLICY CHANGE
Pricing and policies are subject to change. This includes, but is not limited to a price increase in membership costs. %%INITIAL%%

PHOTOGRAPHY Our instructors + Village Coordinators often take pictures/videos throughout class, during crafts, and at FIT4MOM events. Photos/videos are used for social media + marketing materials for our village. If you do NOT want photos of yourself or your child(ren) taken, please send a written request to melissamerrill@fit4mom.com. Unless a written request has been received, by agreeing to these terms you are giving FIT4MOM permission to use photos from class and other FIT4MOM events online. %%INITIAL%%

AGREEMENT
By purchasing this contract, you are agreeing to the terms of this contract. Terms of this agreement may change with proper advance written notice. %%INITIAL%%
